Missed a dose of Ozempic, Wegovy or Zepbound? What the labels say

Forgot your weekly shot? Each US label has a short rule for what to do, and the rules are not the same for every medicine. Here they are side by side, along with what the labels say about missing two weeks or more.

The short version
  • The US labels allow a late weekly dose within 5 days for Ozempic and 4 days for Zepbound and Mounjaro. Wegovy injection can be taken late if the next dose is more than 2 days away. Past that, each label says to skip it.1–4
  • Only the Wegovy label covers missing 2 or more doses in a row: it says to restart the dose escalation at a lower dose to lower the risk of stomach side effects.1
  • This is what the labels say, not advice for you. Confirm with your prescriber or pharmacist, especially after a longer break.

What each label says about a missed dose

Semaglutide (Ozempic, Wegovy) and tirzepatide (Zepbound, Mounjaro) are taken once a week. Each US label has a missed-dose rule in its Dosage and Administration section (section 2) and repeats it in the patient information. These are the label's instructions, quoted as closely as plain English allows.

MedicineLate dose allowedIf later than thatMissed 2+ weeks
Wegovy injection (semaglutide)If the next scheduled dose is more than 2 days (48 hours) away, take it as soon as possibleIf the next dose is less than 2 days away, skip it and take the next dose on the usual dayRestart dose escalation at a lower dose; contact your provider about how to restart
Wegovy tablets (oral semaglutide, daily)No late doseSkip the missed tablet and take the next one the following dayNot addressed
Ozempic (semaglutide)Within 5 days after the missed doseAfter more than 5 days, skip it and take the next dose on the usual dayNot addressed
Zepbound (tirzepatide)Within 4 days (96 hours) after the missed doseAfter more than 4 days, skip it and take the next dose on the usual dayNot addressed
Mounjaro (tirzepatide)Within 4 days (96 hours) after the missed doseAfter more than 4 days, skip it and take the next dose on the usual dayNot addressed

On a 7-day schedule, the two semaglutide rules work out to about the same window. "Within 5 days of the missed dose" and "more than 2 days before the next one" describe nearly the same stretch of time. In every case, the labels say to go back to the regular once-weekly day afterward. None of them says to take two doses to catch up.

How close together can two doses be?

The labels also set a minimum gap, which matters if you take a late dose or move your dose day:

2 daysminimum between doses when changing the dose day, Ozempic and Wegovy (48 hours)
3 daysminimum between doses, Zepbound and Mounjaro (72 hours)

The Zepbound and Mounjaro patient information puts it plainly: do not take 2 doses within 3 days of each other.3,4 The Ozempic label allows a change of dose day as long as at least 2 days have passed since the last dose, and the Wegovy patient guide says the same.1,2

Common situations, and the matching label rule

  • Missed Zepbound or Mounjaro by 2 or 3 days. That is within the 4-day window, so the label says to take it as soon as possible and keep the usual day after that.3,4
  • Missed Zepbound by 5 days or more. Past 4 days, the label says to skip the dose and wait for the next scheduled day.3
  • Missed Ozempic for a whole week. More than 5 days have passed, so the label says to skip it and take the next dose on schedule.2
  • Missed 2 weeks of Wegovy. The label says to restart escalation at a lower dose, and its patient guide says to contact your provider.1
  • Missed 2 weeks or more of Ozempic, Zepbound or Mounjaro. These labels do not give a specific rule. Ask your prescriber before the next dose.

Why the windows differ: half-life

The windows track how long each drug stays in the body. Each label's Clinical Pharmacology section gives the elimination half-life, the time for the level in the blood to fall by half:

MedicineHalf-life (label)What the label adds
Semaglutide (Wegovy, Ozempic)About 1 weekPresent in the blood for about 5 to 7 weeks after the last Wegovy 2.4 mg injection; about 5 weeks after the last Ozempic dose
Tirzepatide (Zepbound, Mounjaro)About 5 days (5 to 6 days in people with overweight or obesity)The Mounjaro label says this half-life enables once-weekly dosing

Semaglutide lasts a bit longer, which fits its slightly wider late-dose window. Because levels fall slowly, one missed week does not mean the drug is gone. But levels do drop, and after a longer gap the body is less used to the medicine.

Why restarting after a break can bring side effects back

Nausea, vomiting and other stomach side effects are most common when people start or step up a GLP-1 dose. The Wegovy label ties its 2-week rule to exactly this: it says to restart at a lower dose "to reduce the risk of gastrointestinal adverse reactions."1 In the Wegovy label's adult weight-loss trials, severe stomach side effects were reported in 4.1% of people on Wegovy injection vs 0.9% on placebo.1

A 2026 modeling paper by Novo Nordisk researchers notes that people usually develop tolerance to these stomach effects with regular use, which is why GLP-1 medicines come with guidance on resuming after missed doses.5 It is a simulation, not a trial, and its authors work for the maker of semaglutide.

The Ozempic, Zepbound and Mounjaro labels do not spell out a restart plan after a long gap. That does not mean restarting at the old dose is fine. It means the decision is left to your prescriber, who knows your dose, how long you have been off and how you tolerated it before. This article summarizes the US labels. It is not personal medical advice. Labels in other countries can differ, and your prescriber may give you different instructions. If you are unsure whether to take a late dose, call your prescriber or pharmacist. If you have type 2 diabetes and also take insulin or a sulfonylurea, ask how a missed or late dose affects your blood sugar plan.

Making missed doses less likely

Weekly medicines are easy to forget because the routine only comes once every seven days. A fixed dose day, a phone reminder and a written log of each dose all help. A log also answers the questions your prescriber will ask: when was your last dose, and how many have you missed? It is also a good moment to note where you injected; see where to inject Ozempic, Wegovy or Zepbound.

Common questions

What happens if I miss my Ozempic dose for 1 week?

The US Ozempic label says to take a missed dose as soon as possible within 5 days after the missed dose. If more than 5 days have passed, the label says to skip it and take the next dose on the regular day, then continue once weekly.

I missed my Zepbound dose by 2 or 3 days. What does the label say?

The US Zepbound and Mounjaro labels say a missed dose can be taken as soon as possible within 4 days (96 hours). After more than 4 days, they say to skip it and take the next dose on the usual day, and never to take 2 doses within 3 days (72 hours) of each other.

What if I missed 2 weeks of Wegovy?

The US Wegovy label says that if 2 or more doses in a row are missed, the dose escalation should be restarted at a lower dose to reduce the risk of stomach side effects. Its patient guide says to contact your healthcare provider about how to restart.

What do the labels say about restarting Ozempic or Zepbound after a break?

The current US Ozempic, Zepbound and Mounjaro labels give rules for a single missed dose but no specific instruction for missing 2 weeks or more. Only the Wegovy label does, so after a longer break the restart plan is a question for your prescriber.

What if I miss a Wegovy pill?

The US Wegovy label says that if a daily Wegovy tablet is missed, skip that dose and take the next one the following day. It also says not to take more than one tablet a day.
