- Probably yes for many people: in a 2026 survey of 550 US adults on injectable semaglutide, the median food noise score fell from 13 to 6 out of 20, though "before" scores were recalled from memory.10
- Randomized trials of semaglutide and tirzepatide consistently found less hunger, fewer cravings and better control of eating than placebo. They measured cravings, not "food noise" by name.5–8
- No published study has tested whether food noise returns late in the dosing week. That question is still open.
What does food noise mean?
"Food noise" started as an informal phrase on social media and in clinics, not a medical term. Researchers began studying it after patients on semaglutide described less rumination and preoccupation about food.2 The first validated tool, the 5-item Food Noise Questionnaire (FNQ), defines it as persistent, intrusive thoughts about food that are disruptive to daily life and make healthy behaviors difficult. Scores run from 0 to 20.1,10
How common is it? It depends on how you ask:
- In a nationally representative survey of 1,000 US adults in May 2026, using a single question, 50.9% said they experience food noise at least occasionally and 18% frequently. Frequent food noise was more common in current GLP-1 users (25.8%) than non-users (16.3%), and was most strongly linked to loss of control over eating.3
- A larger survey of 3,646 US adults using the full FNQ put the share who clearly meet criteria at about 4% to 9.5%, depending on the cutoff. This is a preprint that has not yet been peer reviewed.4
The higher rate among GLP-1 users does not mean the drugs cause food noise. People who struggle most with food thoughts may be more likely to seek treatment. These surveys capture one moment and cannot show cause.

Does GLP-1 reduce food noise? What the studies found
The large weight-loss trials were designed before "food noise" was a research term. Instead they used the Control of Eating Questionnaire, which asks about hunger, fullness, cravings and how hard cravings are to resist.
| Study | Who | Main finding on appetite and cravings |
|---|---|---|
| Semaglutide 1.0 mg, 12 weeks, crossover trial (2017) | 30 adults with obesity | 24% less energy eaten across all test meals vs placebo; less hunger and food cravings, better control of eating |
| Semaglutide 2.4 mg, 20 weeks, randomized (2021) | 72 adults with obesity | 35% less energy eaten at a test lunch; fewer and weaker food cravings |
| Semaglutide 2.4 mg, 104 weeks, STEP 5 (2023) | 174 adults with overweight or obesity who completed the questionnaire | Better craving control and fewer savory cravings at weeks 20, 52 and 104 |
| Tirzepatide, 6 weeks, randomized (2025) | 114 adults without diabetes, BMI 27 to 50 | About 525 kcal less eaten at a test lunch at week 3; fewer cravings, less reactivity to food cues |
| INFORM survey (2026) | 550 US adults on injectable semaglutide | Median FNQ score 13 recalled before vs 6 after starting |
Semaglutide (Wegovy, Ozempic)
In a 12-week crossover trial of 30 adults with obesity, semaglutide dose-escalated to 1.0 mg led to 24% less energy eaten across all test meals in a day than placebo. People reported less hunger and fewer food cravings, better control of eating and a lower preference for high-fat foods, while nausea ratings were similar.5 A 20-week trial of semaglutide 2.4 mg in 72 adults with obesity found 35% lower intake at an all-you-can-eat lunch and fewer, weaker cravings than placebo.6
The longest data come from STEP 5. Among 88 people on semaglutide 2.4 mg and 86 on placebo who filled in the questionnaire, craving control and savory cravings were better on semaglutide at 20, 52 and 104 weeks. Sweet cravings improved at 20 and 52 weeks. Hunger and fullness differed from placebo only at week 20. Bigger craving improvements tracked with more weight lost (−14.8% on semaglutide vs −2.4% on placebo).7 In other words, the effect on cravings seemed to last longer than the effect on plain hunger.
Tirzepatide (Zepbound, Mounjaro)
In a 6-week trial of 114 adults without diabetes, tirzepatide cut energy intake at a test lunch by about 525 kcal vs placebo at week 3. It reduced overall appetite, food cravings, tendency to overeat and reactivity to food in the environment. Brain scans showed lower activity in reward-related areas when people looked at high-fat, high-sugar food photos.8 In a 28-week trial in people with type 2 diabetes, tirzepatide 15 mg and semaglutide 1 mg both reduced appetite vs placebo, with no clear difference between them.9 For more on the two drugs, see semaglutide vs tirzepatide.
The first study to measure "food noise" by name
The INFORM survey asked 550 US adults using injectable semaglutide for weight management to complete the FNQ. Most were women (86%) and most had used semaglutide for at least 4 months. The median score was 13 out of 20 for how they remembered feeling before treatment and 6 now. Agreement with statements like "I spend too much time thinking about food" fell from 47–63% to 15–20%.10
Why does food noise come back late in the week?
Many people say cravings creep back in the day or two before their next weekly shot. We searched PubMed and Europe PMC and found no published study that has measured food noise or cravings across the days of the dosing week, so the evidence is limited.
What the labels do say: semaglutide (Wegovy) has an elimination half-life of about 1 week, and tirzepatide (Zepbound) about 5 to 6 days. Tirzepatide levels reach a steady state after about 4 weeks of weekly dosing.13 These long half-lives are why the drugs are taken once a week. Whether drug levels dip enough within a week to change appetite for some people has not been tested.
Other things can make food thoughts louder too: stress, poor sleep, skipped meals and weekend routines. If you notice a pattern, a simple daily note on appetite and cravings next to your dose day can make the conversation with your prescriber more concrete. Do not change your dose or schedule on your own. If you miss a dose, see what the labels say about a missed GLP-1 dose.
Can food noise come back while you are still on a GLP-1?
The data here are thin. Two small studies show it is not always quiet:
- One person with a brain implant (2025). Researchers recording from the nucleus accumbens, a reward area of the brain, in a woman with obesity and loss-of-control eating saw almost no severe food preoccupation episodes in months 2 to 4 after surgery while she was on tirzepatide. In months 5 to 7, a brain signal linked to preoccupation reappeared and breakthrough episodes returned despite the maximum tirzepatide dose.12 This is a single case.
- A small real-world cohort (2026). Among 28 adults with obesity starting semaglutide or tirzepatide without structured nutrition support, food cravings were unchanged overall at 24 weeks. People with the highest cravings at the start had the biggest drops.11
After stopping the medicine, appetite and weight tend to come back. See what happens when you stop a GLP-1. GLP-1s may also affect other cravings, such as for alcohol; see GLP-1 and alcohol.
Common questions
Does GLP-1 reduce food noise?
In the 2026 INFORM survey of 550 US adults using injectable semaglutide for weight loss, the median Food Noise Questionnaire score fell from 13 (recalled, before treatment) to 6 out of 20. Randomized trials did not use the term food noise, but in the 2-year STEP 5 trial semaglutide 2.4 mg improved craving control compared with placebo.
What does food noise mean?
Food noise is an informal term, not a diagnosis. The first validated questionnaire defines it as persistent, intrusive thoughts about food that disrupt daily life and make healthy behaviors difficult. In a 2026 nationally representative survey of 1,000 US adults, 18% said they experience it frequently.
Why does food noise come back on dose day or late in the week?
No published study has measured whether food noise rises in the days before a weekly GLP-1 injection, so the evidence is limited. The US labels give an elimination half-life of about 1 week for semaglutide (Wegovy) and about 5 to 6 days for tirzepatide (Zepbound). If you notice a weekly pattern, it is worth raising with your prescriber.
Does tirzepatide reduce food cravings?
In a 6-week randomized trial of 114 adults without diabetes and a BMI of 27 to 50, tirzepatide (Zepbound, Mounjaro) cut energy intake at a test lunch by about 525 kcal compared with placebo at week 3. It also reduced food cravings, hunger and reactivity to food in the environment.
Can food noise come back while still on a GLP-1?
It can, at least in some people. A 2025 report of one woman with a brain implant described almost no severe food preoccupation episodes in months 2 to 4 on tirzepatide, then breakthrough episodes in months 5 to 7 despite the maximum dose. One case cannot show how common this is.
