- The US Ozempic label starts at 0.25 mg weekly for 4 weeks, then 0.5 mg, with steps to 1 mg and a maximum of 2 mg weekly. Wegovy climbs to 2.4 mg, with a maximum of 7.2 mg weekly for adults losing weight.1,2
- Ozempic and Wegovy contain the same medicine but are different products, for different uses, with different pens and different maximum doses. The tablets (Rybelsus, Ozempic tablets, Wegovy tablets) have their own doses again.1–3
- The approved pens are dosed in milligrams, not units or clicks. The FDA has reported people taking 5 to 20 times the intended dose of compounded semaglutide after confusion over units.4
Semaglutide dosage chart: every US product at a glance
Semaglutide is in five US products, each with its own starting dose, steps and maximum in section 2 of its label. This chart shows what the labels say as of November 2026. It is not a dosing plan.
| Product | Approved for (adults unless noted) | Starting dose | Highest dose on the label |
|---|---|---|---|
| Ozempic injection, weekly | Type 2 diabetes, including heart and kidney risk reduction | 0.25 mg | 2 mg weekly |
| Wegovy injection, weekly | Weight reduction (also ages 12 and up), heart risk reduction, MASH | 0.25 mg | 7.2 mg weekly (adult weight reduction only) |
| Wegovy tablets, daily | Weight reduction and heart risk reduction | 1.5 mg | 25 mg daily |
| Rybelsus tablets, daily | Type 2 diabetes | 3 mg | 14 mg daily |
| Ozempic tablets, daily | Type 2 diabetes | 1.5 mg | 9 mg daily |

Ozempic dosage chart
The Ozempic label (section 2.2) says to start at 0.25 mg once weekly for 4 weeks, "to reduce the risk of gastrointestinal adverse reactions," then increase to 0.5 mg. After that, each step needs at least 4 weeks on the current dose, and only if more blood sugar control is needed.1
| Earliest weeks | Once-weekly Ozempic dose | What the label says |
|---|---|---|
| 1 to 4 | 0.25 mg | Starting dose |
| 5 onward | 0.5 mg | Maintenance option |
| 9 onward, if needed | 1 mg | Maintenance option; the maintenance dose for type 2 diabetes with chronic kidney disease |
| 13 onward, if needed | 2 mg | Maintenance option; maximum dose |
Ozempic comes in two forms, according to section 3 of the label. The first is a multi-dose pen for one person that holds about a month of doses: one pen gives 0.25 mg and 0.5 mg doses (2 mg in 3 mL), another gives 1 mg doses (4 mg in 3 mL) and another gives 2 mg doses (8 mg in 3 mL). The second is a single-dose prefilled syringe in 0.25 mg, 0.5 mg and 1 mg.1 The label says an Ozempic pen must never be shared between people, even if the needle is changed.
What is the highest dose of Ozempic?
The label says "the maximum recommended dosage is 2 mg once weekly."1 It describes a 40-week trial of 961 adults with type 2 diabetes on metformin, with or without a sulfonylurea, who were assigned to 2 mg or 1 mg:
HbA1c is a blood test of average blood sugar over about 3 months. In this trial, 64% on 2 mg and 56% on 1 mg reached an HbA1c below 7%.1
What is the maximum dose of Ozempic for weight loss?
Ozempic is not approved for weight loss in the US. Its label lists uses in adults with type 2 diabetes only, and its maximum is 2 mg weekly.1 The semaglutide product approved "to reduce excess body weight and maintain weight reduction long term" is Wegovy, which has its own pens, its own doses and a higher maximum.2 For a side-by-side comparison, see Ozempic vs Wegovy vs Mounjaro vs Zepbound.
Wegovy injection dosage chart
The Wegovy label (section 2.2, Table 1) uses fixed 4-week steps for every approved use:2
| Weeks | Once-weekly Wegovy dose | Stage |
|---|---|---|
| 1 to 4 | 0.25 mg | Starting dose |
| 5 to 8 | 0.5 mg | Escalation |
| 9 to 12 | 1 mg | Escalation |
| 13 to 16 | 1.7 mg | Escalation |
| 17 onward | 1.7 mg or 2.4 mg (2.4 mg recommended for adult weight reduction) | Maintenance |
| After at least 4 weeks on 2.4 mg | Up to 7.2 mg, if more weight reduction is clinically indicated | Maximum, adult weight reduction only |
For heart risk reduction and for children 12 and older, the label lists 2.4 mg (recommended) or 1.7 mg; for MASH it lists 2.4 mg. The 7.2 mg dose comes as a single-dose pen sold as Wegovy HD. The other strengths come as single-dose pens or syringes, and a Wegovy FlexTouch pen holds four 2.4 mg doses.2 The full week-by-week detail, including what the label says if a step is hard to tolerate, is in our Wegovy and Zepbound dose schedule, and the trial results for the top dose are in Wegovy 7.2 mg.
Semaglutide tablet dosage chart
Three semaglutide tablets are on US labels as of November 2026. All three are taken once daily, in the morning on an empty stomach with up to 4 ounces of water, followed by a wait of at least 30 minutes before eating, drinking or taking other oral medicines. Each label climbs in 30-day steps:2,3
| Days | Wegovy tablets | Rybelsus | Ozempic tablets |
|---|---|---|---|
| 1 to 30 | 1.5 mg | 3 mg | 1.5 mg |
| 31 to 60 | 4 mg | 7 mg | 4 mg |
| 61 to 90 | 9 mg | 7 mg, or 14 mg if more blood sugar control is needed | 4 mg, or 9 mg if more blood sugar control is needed |
| 91 onward | 25 mg (maintenance) | 7 mg or 14 mg | 4 mg or 9 mg |
Two label details stand out. The Rybelsus and Ozempic tablets label says the 3 mg and 1.5 mg starting doses are "not effective for glycemic control," so they exist only to let the body adjust. It also says Rybelsus and Ozempic tablets "are not substitutable on a mg-to-mg basis": 7 mg Rybelsus lines up with 4 mg Ozempic tablets, and 14 mg with 9 mg.3 Tablet doses are also far higher than injection doses because little of a swallowed dose is absorbed: the Wegovy label estimates about 1% to 2% for tablets vs 89% for injection. It says average blood levels on 25 mg tablets daily and 2.4 mg injection weekly were similar in people without type 2 diabetes.2 So a 25 mg tablet is not a "stronger" dose than a 2.4 mg shot. For the weight-loss trials of the pill, see oral semaglutide for weight loss.
Why every chart starts low
Each label ties its step-up schedule to stomach side effects, and the label data show why:
The Ozempic label says most nausea, vomiting and diarrhea happened during dose escalation, and 3.1% on 0.5 mg and 3.8% on 1 mg stopped treatment because of them, vs 0.4% on placebo.1 The Wegovy label also says these reactions were reported most often during dose escalation.2 For more, see GLP-1 nausea and stomach side effects. If doses are missed, the labels have separate rules; see missed a dose.
Ozempic click dose chart and semaglutide dosage in units
Many people search for a "click chart" or a dose "in units." The FDA-approved labels do not use either.
- Units. The Ozempic and Wegovy labels list doses in milligrams (mg) and volumes in milliliters (mL). They do not give doses in units.1,2 "Units" are the markings on an insulin-type syringe, and they measure volume, not how much drug is in it.
- Clicks. The Ozempic pen's Instructions for Use say the dose selector clicks as it turns, and tell users not to set the dose by counting clicks: "Do not count the pen clicks." They say to use the dose counter and dose pointer, which show the dose in mg.1 Wegovy single-dose pens deliver one fixed dose, so there is nothing to dial.2
- Compounded semaglutide. Compounded products are mixed by pharmacies and are not FDA-approved. They come in vials and syringes at concentrations that vary between pharmacies, and one pharmacy may offer several. So the same number of units can contain different amounts of semaglutide depending on the vial.4,5
In a July 2024 alert, the FDA described people who measured compounded semaglutide from multi-dose vials taking 5 to 20 times the intended dose, and prescribers who miscalculated doses giving 5 to 10 times the intended dose. The causes included confusion between milliliters, milligrams and units, and unfamiliarity with drawing medicine from a vial. Some people needed hospital care for problems including vomiting, dehydration, fainting, pancreatitis and gallstones.4 As of May 31, 2026, the FDA had received 990 adverse event reports linked to compounded semaglutide.5 More detail is in compounded semaglutide and tirzepatide.
This article summarizes the current US labels. It is not a dosing plan, and labels in other countries can differ. Your prescriber decides your starting dose, when or whether to step up, and your maintenance dose. Common questions
What is the Ozempic dosage chart?
The US Ozempic label lists 0.25 mg once weekly for 4 weeks, then 0.5 mg. If more blood sugar control is needed after at least 4 weeks on a dose, it may go to 1 mg and then 2 mg, and 2 mg once weekly is the maximum. Ozempic is approved for adults with type 2 diabetes.
What is the maximum dose of Ozempic for weight loss?
Ozempic is not approved for weight loss in the US; its label covers type 2 diabetes, and its maximum is 2 mg once weekly. The semaglutide product approved for weight reduction is Wegovy, whose label allows up to 7.2 mg weekly in adults who have tolerated 2.4 mg for at least 4 weeks.
What is the highest dose of Ozempic?
The US Ozempic label lists 2 mg once weekly as the maximum recommended dose. In a 40-week trial in the label of 961 adults with type 2 diabetes, 2 mg lowered HbA1c by 2.1 points vs 1.9 on 1 mg.
What is the Ozempic starting dose?
The US Ozempic label says to start at 0.25 mg injected once weekly for 4 weeks and then increase to 0.5 mg. The label says the slow start is meant to reduce the risk of stomach side effects.
What is semaglutide dosage in units?
The US labels for Ozempic and Wegovy give doses only in milligrams, and their pens deliver set doses. Compounded semaglutide comes in different concentrations, so the same number of syringe units can hold different amounts of drug, and the FDA has warned of people taking 5 to 20 times the intended dose.
